Funeral services for Mr. James Chadwick "Chad"� Flatt, 35, of Cookeville will be held Sunday April 22 at 1 p.m. in the Cookeville Chapel of Hooper-Huddleston & Horner Funeral Home. Interment will follow in Curry"�s Chapel Cemetery in Jackson County. Bro. Johnny Fox will officiate. Mr. Flatt died Wednesday, April 18, 2012 in the Emergency Room at Cookeville Regional Medical Center. Mr. Flatt was born December 7, 1976 to James Howard Flatt of Cookeville and Anna "Ann"� Elizabeth Sliger Flatt of Algood. He was a sales representative with Capital City Bolt & Screw Company in Nashville. Chad loved the outdoors and especially loved camping. He was an avid deer hunter. He was preceded in death by his maternal grandfather, Jimmy J. Sliger and paternal grandparents, Sewell & Ruby Flatt. Survivors in addition to his parents include two sisters and a brother-in-law, Sheila & James Hines of Whitleyville, TN, and Sherry Hooten of Algood; maternal grandmother, Mary Lee Sliger of Algood; neice, Makala Ann Hooten; fiancée, Alicia Adair; several aunts & uncles and extended family and his favorite canines, Perue & Persia.
